Course Description

SON2171C – Vascular Sonography covers ultrasound imaging and Doppler evaluation of the arterial and venous systems. Daytona State College titles it Vascular Sonography and Lab. The C suffix reflects an integrated scanning laboratory; institutions also carry the content as SON2171 (3 credits) plus SON2171L (1 credit).

Vascular sonography is a distinct specialty within diagnostic medical sonography and one of the more technically demanding. Where abdominal or obstetric sonography largely images structure, vascular work images flow — direction, velocity, waveform shape, and turbulence — and the diagnosis frequently rests on measurements the sonographer must obtain correctly in real time. A poorly angled Doppler sample produces a wrong velocity, and a wrong velocity produces a wrong stenosis grade.

Content covers vascular anatomy — the cerebrovascular, upper and lower extremity arterial and venous, and abdominal vascular systems; hemodynamics — pressure, flow, resistance, and the physics governing them; Doppler principles — the Doppler shift, angle dependence, spectral analysis, color and power Doppler, and aliasing; instrumentation and optimization for vascular work; carotid and vertebral duplex — protocol, velocity criteria, and stenosis grading; transcranial Doppler where included; venous duplex — deep venous thrombosis evaluation, compression technique, and augmentation; venous insufficiency and reflux testing; peripheral arterial evaluation — ankle-brachial index, segmental pressures, and duplex; abdominal vascular — aorta, renal, mesenteric, and portal systems; dialysis access and graft evaluation; pathology and its sonographic appearance; and documentation and preliminary findings.

Sonography is credential-driven. The ARDMS Registered Vascular Technologist (RVT) is the specialty credential for this work, earned by passing the Sonography Principles and Instrumentation (SPI) examination plus the Vascular Technology specialty examination. Cardiovascular Credentialing International offers a parallel RVS credential. Most Florida employers require or strongly prefer credentialed sonographers, and many require the credential within a defined period after hire. Graduation from a CAAHEP-accredited program is the cleanest eligibility route — confirm the program's accreditation before enrolling, because eligibility pathways for non-accredited graduates are longer and more restrictive.

Note that Florida does not license sonographers as it licenses radiographers — there is no state credential to obtain. The national registry is the credential, which makes it correspondingly more important.

Doppler angle is where measurements go wrong

The single most consequential technical skill in vascular sonography. Velocity measurements depend on the cosine of the angle between the beam and the direction of flow, so error grows rapidly as the angle increases — which is why protocols specify keeping it at or below 60 degrees and why angle correction must be aligned to the vessel wall rather than guessed. Since stenosis grading rests on velocity criteria, an angle error becomes a diagnostic error. Students should treat this as the thing to get right before anything else.

Peer scanning is how the skill is built — and it has its own etiquette

Sonography students practice on each other, which means being scanned as well as scanning. Programs set expectations about draping, consent, and professionalism, and students should take them seriously. It is also worth knowing that peer scanning occasionally reveals incidental findings in a healthy classmate; programs have procedures for that, and students should follow them rather than speculating aloud.

Scanning is physically demanding, and career-limiting injuries are common

Work-related musculoskeletal injury is a serious and well-documented occupational problem in sonography — sustained transducer pressure, awkward reach, and static posture produce shoulder, neck, and wrist injuries that end careers. Ergonomics is not a throwaway topic here. Students should learn correct room and equipment setup, patient positioning that brings the anatomy to them, and the habit of adjusting the environment rather than contorting to reach. The SDMS publishes industry standards for ergonomics; they are worth reading early rather than after symptoms start.

Count contact hours, not credits

Institutions carry this as SON2171 (3 credits) plus SON2171L (1 credit), or combined as SON2171C at 4 credits — and the laboratory runs at roughly two contact hours per credit, with clinical courses running far higher.
