The organizing idea, and it is worth laying out concretely because students trained on one setting assume the rules are universal.
Each care setting has its own code sets, payment system, documentation requirements, and coding guidelines, and they genuinely differ:
- Acute inpatient — ICD-10-CM and ICD-10-PCS, paid through MS-DRGs, where a single complication or comorbidity can move a case into a substantially higher-paying group.
- Hospital outpatient and ambulatory surgery — ICD-10-CM with CPT/HCPCS, paid through APCs; no PCS at all.
- Physician office and clinic — ICD-10-CM with CPT/HCPCS, paid on the physician fee schedule using RVUs, with evaluation and management level selection as the dominant daily decision.
- Emergency department — both a facility component and a professional component, coded separately under different rules, which surprises nearly every student.
- Skilled nursing — payment driven by a resident assessment instrument, so the assessment schedule and its accuracy drive revenue as much as the codes do.
- Home health — driven by its own assessment instrument and period-based payment.
- Hospice — paid per diem by level of care rather than per service, with eligibility documentation — a certified terminal prognosis and recertification — carrying the compliance weight instead.
The single most important rule difference to hold onto: in outpatient settings you do not code uncertain diagnoses. ⚠ Coding accuracy is a legal exposure, and the pressure runs both directions

Codes are claims for payment from federal programmes. - Medical necessity is separate from coding accuracy. A correctly coded service that was not medically necessary is still not payable, and documentation has to support both.
- Documentation is the boundary. If it is not documented, it did not happen — the oldest rule in the field and still the operative one.
The career point worth naming: coders are sometimes pressured by employers whose revenue depends on the output. Knowing the guidelines well enough to say "the documentation does not support that, and here is the guideline" is what professional integrity looks like in this job, and it is also what protects you personally.
⚠ Post-acute settings are where Florida's jobs are, and they are demographically driven

Expect coding practice on real record types from each setting — this is a hands-on course despite its size — with assessment by coding accuracy against an answer key. A minimum grade of C is required, which is standard in accredited HIM programmes because accreditation depends on demonstrated competency.
How Florida course levels affect transfer
The first digit of an SCNS number denotes the year of offering, not transferability. Courses at the 1000 and 2000 levels transfer transparently between Florida public institutions, and 3000 to 4000 is unproblematic since both are upper division. The boundary that actually matters is 2000 to 3000, where lower-division credit generally cannot satisfy an upper-division requirement.
Transfer needs unusual care in this prefix for the two reasons above: the suffix divergence and the three parallel numbering families, which mean that a course with a matching title at another institution may be a different number, a different credit value, and a different position in the curriculum.
